D0120 includes a periodic oral evaluation and periodontal screening. Periodontal screening may involve PSR or six-points-per-tooth periodontal charting "as indicated".
D0180 is reported when a comprehensive periodontal charting and evaluation is performed on a patient who presents with signs or symptoms of periodontal disease (or risk factors such as smoking or diabetes).
A comprehensive periodontal charting involves more than just six-points-per-tooth periodontal charting. It also includes recording all bleeding points, furcations, recession, tooth mobility, minimal attachments, and periodontal diagnosis. D0180 also requires a dentist to perform the oral exam (i.e., have clinical contact with the patient).In fact, all evaluation codes require physical contact, evaluation, and diagnosis by a dentist.
